There are cases of severe autism that are extremely complex, such as the one posted on YouTube, when you type in autism and self injurious behavior. PBS doesn't work 100% of the time, so you have to be open minded to treat the patient in such a way that it will protect the individual from acute bodily injury...as seen in the you tube videos on autism and self injury.
